b.well Connected Health and MyDirectives Partner to Bring Advance Care Planning Documents Into Consumer Health Records

Collaboration makes critical advance care planning documents easier for individuals to access and share with the people they trust

BALTIMORE and DALLAS, Oct. 21, 2026 /PRNewswire/ — b.well Connected Health and MyDirectives today announced a partnership to help consumers access their advance care planning documents alongside the rest of their health information. The collaboration will connect b.well’s data network to the MyDirectives national registry, giving people another way to reach documents that record their wishes for care.

MyDirectives maintains the largest nationwide registry of advance care planning documents, including advance directives and portable medical order forms such as POLST and MOLST. These documents make patient goals of care and treatment preferences clear to care teams when patients are unable to communicate for themselves. This matters most at moments when finding this information quickly counts. Too often, these documents are kept in a file cabinet, on a bookshelf, or in a safe deposit box, separate from the rest of a person’s health information, making them harder for the person and their care team to find and share when they’re needed.

Through the partnership, b.well plans to let consumers view their advance care planning documents inside the digital health experiences they already use, in the same place as the rest of their health information. Advance care planning documents would become a new type of information available to consumers in their b.well-powered health summary, and will be available via b.well’s CMS Aligned Network.

“Advance care planning documents reflect a person’s own choices about the care they want, and those choices should be easy to find when they matter most,” said Kristen Valdes, CEO and Founder of b.well Connected Health. “People should be able to access their health information, decide who can see it, and trust that their wishes will be respected. That’s what putting consumers in control of their health information is all about.”

According to MyDirectives, the partnership gives b.well’s users yet another means of taking an active role in their healthcare. “This partnership provides vital health information for b.well users and their care teams,” said Scott Brown, Chief Executive Officer of MyDirectives. “MyDirectives supports CMS’ Health Tech Ecosystem’s focus on helping people access and share health information through digital tools, and we believe everyone should be able to document, manage, and share their goals of care and treatment preferences before a crisis. MyDirectives.care, our free consumer platform, addresses the first part, and apps powered by b.well are another powerful way to make those documents easy for care teams to find.”

The partnership is an early step in extending patient access to advance care planning documents. b.well is the first consumer-facing organization to integrate with MyDirectives’ nationwide registry for a patient access use case. Most existing MyDirectives integrations support advance care planning facilitator access to its MyDirectives for Clinicians solution or clinicians and emergency first responders retrieving documents for treatment. The companies share a goal of making advance care planning information easier to find through the digital tools consumers use every day.
